Q. How can I access my medical records?
A. You can obtain your medical records once you have completed a Release Form. Our office offers the convenience of a flashdrive, you can store, research, and share information that is part of your Electronic Medical Record.
Q. How can I get a refill of my medications?
A. Most patients can call direct to their pharmacy when they need a prescription refilled. The pharmacy contacts our office and our staff processes your request quickly.
Q. How do I make an appointment?
A. If you a new patient to the practice, please call (941) 629-5757 and enter extension 347 to speak to the Office Coordinator. Please make sure you have your insurance information available.

Q. My Doctor is not there today, can I still be seen?
A. If your Doctor is not in and you need to be seen that day, we will make every effort to have you seen by another provider. Please call the Appointment Scheduler at (941) 629-5757 and press 4.
Q. Why do I need to tell the Appointment Scheduler the reason for my visit?
A. Our Appointment Scheduler will need to know some basic information to be certain the right amount of time will be provided on the schedule for your visit.
Q. How do I contact my Doctor after hours?
A. After office hours, weekends and holiday call (941) 629-5757 and a live operator will answer your call. Our physicians are on call 24 hours per day, 7 days per week.
